USA Hockey Concussion Information
USA Hockey requires that if a player exhibits any signs symptoms or behaviors consistent with a concussion, or is suspected of sustaining a concussion they are immediately removed from any training practice or game.
Beginning with the 2019-20 season, any athlete held out for concussion evaluation or who has been diagnosed with a concussion must provide a written Return to Play form from a qualified medical provider allowing them to return to any training, practice, or game activity with no restrictions.
The parent must sign the form and the coach must also sign the form acknowledging that they received it. Once the coach has received the form, it must be filed with the SMS Board of Directors.
If a player has received a concussion through another sport, coaches should request a letter from their qualified medical provider clearing them for unrestricted activity.
